Trial: twice daily for 16 weeks
Who: 12 healthy elderly adults (Manchester trials)
Note: GH response dropped by week 4 and stayed lower at week 16, then returned to baseline 4 weeks after stopping.

Who: 12 healthy elderly adults (Manchester trials)
Note: GH response dropped by week 4 and stayed lower at week 16, then returned to baseline 4 weeks after stopping.
Who: 6 healthy young men
Note: Two and three shots raised 24-hour GH by the same amount, with no change in prolactin, ACTH or cortisol.
Who: Healthy adult volunteers in 1994 dose studies
Note: Used to measure GH release, not for ongoing use. GH peaked about 30 minutes after the dose, and 2 mcg/kg was close to the maximum response.
Who: 8 prepubertal short children aged 4 to 11.6 years
Note: Pediatric trial only: a per-kg dose for prepubertal children, not an adult protocol. Nasal absorption is low (about 4.8% vs 77% subcutaneous in a separate adult study), which is why the nasal dose is so much higher per kg.
Who: Bodybuilding forum protocols compiled in a 2026 peer-reviewed review
Note: Used to check response and side effects before moving up to the standard range.
Who: PeptIQ app users, 24 community reports
Timing: Fasted: before breakfast and/or before bed, 2 to 3 hours after the last meal.
Note: Guides treat about 200 mcg per shot as the ceiling: above it, cortisol keeps rising while GH does not.
Timing: Empty stomach, 2 or more hours after eating: morning, after training, before bed.

Calculate with this peptideYour vial, your water, your dose: Hexarelin in syringe units, with the math shown.Diluent: Bacteriostatic water
Water volumes sources use: 1 mL, 2 mL
Use bacteriostatic water, not saline, which one guide says can make it precipitate. The solution should be clear; do not use it if cloudy.
100 to 200 mcg is the center, 1 to 3 times a day. Forum protocols in a 2026 review run 100 to 200 mcg once or twice daily. Guides treat about 200 mcg as the ceiling because cortisol keeps climbing past it while GH does not.
With a 5 mg vial mixed with 2 mL of bacteriostatic water (2.5 mg/mL), 100 mcg is 0.04 mL, or 4 units on a U-100 insulin syringe. Use the calculator for other mixes.
2 mL is the usual choice for a 5 mg vial, giving 2.5 mg/mL. For a 2 mg vial, 2 mL gives 1 mg/mL. Community users report 1 to 2 mL per 5 mg vial.
Guides disagree. Some keep it short, 4 to 6 weeks on and 2 to 4 off. Others, and forum protocols, run 8 to 12 weeks on with about 4 weeks off. The only long human study ran 16 weeks and saw recovery after 4 weeks off.
