Updated 43 sources

9 side effects reported for P21: 1 from studies and labels, 8 from users and guides. 9 come with the ways people manage them.

Side effects reported in studies and labels

Rates as each trial or label states them. A rate from one trial does not carry over to research-grade vials or other doses.

From trials and regulators1

Clinical studies and regulatory documents.

  • No immune reaction in rats

Reported in aged rats on chronic oral dosing; no human safety data exist

    How people manage it

    • Not applicable; listed because people ask about immune reactions to peptides.

Side effects users and guides report

What people post and what dosing guides list. These are reports, not measured rates.

From users and guides8

Community reports, competitor and vendor guides, media.

  • Sleep disruption or overstimulation

The most consistently mentioned complaint in community reports, mainly at higher doses or with evening doses

    How people manage it

    • Dose in the morning; forums advise against evening doses.
    • Drop to a lower dose.
  • Lethargy, brain fog or flat mood

Occasional user reports, including one daily log at 2 mg a day by nose

    How people manage it

    • The user in the log planned to cut to 500 mcg once or twice a day.
    • Stop and reassess if it persists.
  • Vivid dreams

Described by several users, per one log

    How people manage it

    • No action reported; morning dosing is the usual advice.
  • Headache, irritability or feeling wired

    Mentioned occasionally in community reports

    How people manage it

    • Lower the dose or dose earlier in the day.
  • Nasal irritation (spray)

A non-specific complaint with nasal use

    How people manage it

    • Alternate nostrils; some users switch to injection.
  • Effect fades with daily use

A few users report a strong first week, then less

    How people manage it

    • One user overused sprays chasing the first effect; communities suggest steady, lower doses.
  • Panic, anxiety and feeling cold

One user report, about a week into P21 taken with GHK-Cu

    How people manage it

    • The user stopped all peptides. Episodes recurred for about a week, came back milder after restarting GHK-Cu alone, and some symptoms remained a week after stopping again.
  • Vision problems after long nasal use

One unverified user report after 7.5 months of nasal use; a rodent study found P021 protected the aging retina

    How people manage it

    • Stop and see an eye doctor if glare, halos or blurred vision appear.

How much is known

Animal data only

About a dozen mouse and rat studies, nearly all from the inventor's lab, report more new neurons and BDNF, less tau and better memory in Alzheimer's, aging, Down syndrome and brain-injury models, mostly with P021 mixed into food. A 2024 study led by a lab in Bologna, with the inventor as co-author, saw no BDNF rise in Cdkl5 knockout mice. The inventor co-founded the company developing P021. No human trial is registered.
