Every Cagrilintide protocol on file, 13 in all, labelled by where each number came from. 2 have titration steps, each step is worked out in U-100 units for a 5 mg vial + 2 mL water.
Who: Adults in the 26-week phase 2 dose-finding trial (n=706). Each arm got one fixed target dose. No regulator has approved any cagrilintide dose.
Timing: Arms escalated to their target dose over up to 6 weeks, then held it to week 26.
Who: The highest-dose arm of the phase 2 trial, as laid out by dosing guides that cite it. This is far above what community users report running.
Timing: 4.5 mg from week 7 through the end of the 26-week trial.

Who: Self-reported by posters on r/Biohacking and r/Tirzeglutide who added cagrilintide to a stable GLP-1. Well below the 2.4 mg trial dose. No trial tested this.
Timing: Added after the main GLP-1 is stable. Often injected on a different weekday from the GLP-1, for example two days after retatrutide, to spread out side effects. One poster found 1 mg too strong and stayed at 0.25 to 0.5 mg.
Who: Self-reported by one r/MounjaroMaintenance poster running tirzepatide 2 mg twice a week. No trial tested this.
Who: Self-reported by r/Biohacking posters, mostly alongside retatrutide. No trial tested this.
Timing: Start near 0.25 to 0.3 mg. One poster held 0.3 mg for 4 weeks and then doubled the dose every 4 weeks; another worked up to 1.5 mg weekly.
Who: Self-reported by posters on r/GLP1ResearchTalk and r/Retatrutide. One cut retatrutide from 12 mg to 5 mg and added 2 mg cagrilintide; another ran 2.4 mg with retatrutide 15 mg. No trial tested this.

Very concentrated mixes make small starting doses hard to draw accurately, so guides warn against using too little water in a 10 mg vial. At low add-on doses a 10 mg vial lasts far longer than the usual 28 days after mixing. Blended vials also exist, such as 12.5 mg retatrutide plus 2.5 mg cagrilintide mixed with 2.5 mL. Do not shake. Some users add dilute acetic acid before the water to lower pH; no study supports this.
Dosing guides start at 0.25 mg once a week. The lowest phase 2 trial arm was 0.3 mg. People adding it to a GLP-1 often go lower, around 0.1 to 0.25 mg, because a first 0.25 mg dose on top of a GLP-1 hits some people hard.
Phase 2 tested 0.3, 0.6, 1.2, 2.4 and 4.5 mg once weekly for 26 weeks. CagriSema trials use 2.4 mg cagrilintide with 2.4 mg semaglutide for 68 weeks.

Phase 2 reported 6.0 to 10.8% at 26 weeks across 0.3 to 4.5 mg, against 3.0% on placebo. In REDEFINE 1 the cagrilintide-only arm is reported at 11.8% at 68 weeks in people who stayed on treatment.
