Venous leg ulcer gel (trial)
Who: 34 adults with hard-to-heal venous leg ulcers in the 2014 first-in-man trial.
Note: 0.5 mg/mL healed best (about six times the placebo healing rate); 3.2 mg/mL did no better than placebo.
Also known as: LL37 · LL 37 · Cathelicidin · Human cathelicidin antimicrobial peptide · CAP-18 · hCAP-18 fragment · Ropocamptide

Every LL-37 protocol on file, 18 in all, labelled by where each number came from. 3 have titration steps, each step is worked out in U-100 units for a 5 mg vial + 1 mL water.
Who: 34 adults with hard-to-heal venous leg ulcers in the 2014 first-in-man trial.
Note: 0.5 mg/mL healed best (about six times the placebo healing rate); 3.2 mg/mL did no better than placebo.
Who: 148 adults with hard-to-heal venous leg ulcers (HEAL LL-37).
Note: No significant healing benefit in the full study population; a post hoc look found benefit in ulcers of 10 cm2 or more.
Timing: Most write-ups start at 100 to 200 mcg; 500 mcg is the top of the range.
Note: No human trial has tested LL-37 by injection. These are community figures collected by guides. The community aggregator lists the same 100 to 200 mcg dose over 8 to 12 weeks rather than 2 to 4.
| When | Dose | U-100 units (5 mg vial + 1 mL water) |
|---|---|---|
| Week 1 | 50 mcg | 1 units · 0.01 mL |
| Week 2 | 100 mcg | 2 units · 0.02 mL |
| Week 3 | 150 mcg | 3 units · 0.03 mL |
| Week 4 | 200 mcg | 4 units · 0.04 mL |
| Week 5 | 250 mcg | 5 units · 0.05 mL |
| Week 6 | 300 mcg | 6 units · 0.06 mL |
| Week 7 | 350 mcg | 7 units · 0.07 mL |
| Week 8 | 400 mcg | 8 units · 0.08 mL |
Note: Adds about 50 mcg a week as tolerated, aiming for 200 to 400 mcg by weeks 4 to 8.
| When | Dose | U-100 units (5 mg vial + 1 mL water) |
|---|---|---|
| Week 1 | 100 mcg | 2 units · 0.02 mL |
| Weeks 2 to 8 | 200 mcg | 4 units · 0.04 mL |
Note: Usually stacked with BPC-157 and KPV. Injected, not swallowed, even for gut goals.
Timing: Morning in the guide's weekly chart.
Timing: Watch for a big site reaction, flu-like symptoms or a psoriasis or rosacea flare before continuing.
| When | Dose | U-100 units (5 mg vial + 1 mL water) |
|---|---|---|
| Week 1 | 200 mcg | 4 units · 0.04 mL |
| Weeks 2 to 6 | 500 mcg | 10 units · 0.1 mL |
| Weeks 7 to 8 | 250 mcg | 5 units · 0.05 mL |
Note: The guide pairs it with conventional antimicrobials from a clinician.
Note: The same guide says continuous daily use beyond 16 weeks is uncharacterized.
Note: Top of the range. The guide says doses above 1 mg per injection are not supported.
Note: Some users report flu-like symptoms at these doses.
Note: Stated as 10 to 50 mcg/mL. Not for rosacea or psoriatic skin.
Who: Melanoma patients with skin metastases in a small Phase 1/2 dose-escalation study (4 enrolled).
Note: Starting dose per tumour, injected into 2 to 4 tumours. A cancer trial, not a wellness protocol.

Calculate with this peptideYour vial, your water, your dose: LL-37 in syringe units, with the math shown.Diluent: Bacteriostatic water
Water volumes sources use: 1 mL, 2 mL, 3 mL
Doses are small, so a 30 or 50 unit syringe reads more easily. The solution should be clear and colourless; discard it if cloudy.
Community write-ups cluster at 100 to 250 mcg subQ once daily, often 5 days on and 2 off, for 2 to 4 weeks. Guides start at 25 to 100 mcg and some go to 500 or 750 mcg. No injection dose has been tested in people.
With 5 mg in 2 mL of water, 250 mcg is 10 units on a U-100 syringe. With 5 mg in 1 mL it is 5 units. Use the calculator for other mixes.
Short blocks of 2 to 4 weeks with 2 or more weeks off are the most common. Some guides run 8 to 12 weeks, rarely 16. No cycle length has been validated.
Keep it at 2 to 8 C. Advice runs from 7 days to 30 days, and forum users usually finish a vial in 2 to 3 weeks. It breaks down in solution, so mix only what you will use.
