Type 2 diabetes2

Type 2 diabetes, China label schedule

Amount: 2 to 6 mg
How often: Once weekly: 2 mg for 4 weeks, then 4 mg; 6 mg after at least 4 weeks on 4 mg if more glucose control is needed
Route: SubQ, prefilled pen

Who: Adults in China with type 2 diabetes, alone or added to metformin, sulfonylureas or SGLT2 inhibitors. Taken from the label text in Innovent's reimbursement filing published by China's healthcare security agency.

Timing: The 2 mg dose is a starting step, not a glucose-lowering dose. Maintenance is 4 or 6 mg; the filing says 4 mg is the usual one.

Who: Chinese adults with type 2 diabetes: 320 on diet and exercise alone, and 731 on oral diabetes drugs compared with dulaglutide 1.5 mg.

Timing: On diet and exercise alone, HbA1c fell 1.57 points on 4 mg and 2.15 on 6 mg vs 0.14 on placebo at 24 weeks. Both doses beat dulaglutide on HbA1c and weight at 28 weeks.

Weight loss, phase 3 trial doses (GLORY-1)

Amount: 4 to 6 mg
How often: Once weekly at 4 or 6 mg for 48 weeks
Route: Subcutaneous injection
Cycle: 48 weeks

Who: 610 Chinese adults with a BMI of 28 or more, or 24 to 28 with a weight-related condition.

Timing: Average weight change at 48 weeks: -11.00% at 4 mg and -14.01% at 6 mg, vs +0.30% on placebo. At 6 mg, 49.5% lost at least 15%.

Note: The abstract gives target doses only; the step-up from a lower start is in the full paper.

Weight loss, phase 3 high dose (GLORY-2)

Amount: 9 mg
How often: Once weekly at 9 mg for 60 weeks
Route: Subcutaneous injection
Cycle: 60 weeks

Who: 461 Chinese adults with a BMI of 30 or more, 16.1% with type 2 diabetes.

Timing: Average weight loss at 60 weeks: 16.65% vs 1.50% on placebo. 84.3% lost at least 5%.

Who: 179 US adults with obesity or overweight and no type 2 diabetes.

Timing: Weight loss at 32 weeks: 7.3% on 3 mg, 15.6% on 10 mg and 18.1% on 16 mg, vs 0.9% on placebo. 20% stopped 16 mg because of side effects.

Note: The 3 to 6 mg arm held 3 mg from week 4 to week 32, then moved to 6 mg. The abstract does not give the step schedules for 10 and 16 mg.

Timing: Any time of day, with or without meals, at least 5 days apart. If 6 mg is not tolerated, step back to 4 mg.

Who: Self-reported use outside China. One tracking site sums community reports as 3 to 9 mg weekly with 3 mg as a common first dose. On r/Mazdutide posters start lower, at 1 to 2 mg, and report holding 1.5 to 5 mg. No consensus protocol exists.

Note: A 3 mg start is higher than the 2 mg start on the Chinese label and the 1.5 to 2 mg starts in phase 2. The Reddit logs we found start at 1 to 2 mg and mostly add mazdutide to tirzepatide; see the next entry.

Added to tirzepatide after a stall (community-reported)

Amount: 1 to 6 mg
How often: Once weekly, often 3 to 4 days apart from tirzepatide. Starts at 1 or 2 mg; posters report holding 1.5 to 5 mg

Who: People already on tirzepatide who hit a plateau, as posted on r/Mazdutide. Doses below are mazdutide only; tirzepatide stays at about 4 to 15 mg in these posts.

Note: No trial has tested mazdutide with tirzepatide. One poster who reached 6 mg with tirzepatide 7.5 mg had heavy diarrhea and dropped back to 4 mg.

Mixing Mazdutide

Diluent: Bacteriostatic water

Water volumes sources use: 2 mL, 3 mL

The approved Chinese product is a ready-to-use pen, so mixing only applies to research vials. Swirl, do not shake. At 3.33 mg/mL a 5 mg dose is 1.5 mL, more than a 1 mL insulin syringe holds. One r/Peptides poster reported a mixed vial starting to gel after a few weeks.

If you miss a dose

Per the label as reported: if 5 days or fewer have passed, inject as soon as you can and carry on as planned. If more than 5 days have passed, skip it and take the next dose on schedule.

Mazdutide dosing questions

What is the mazdutide titration schedule?

China's type 2 diabetes label starts at 2 mg weekly for 4 weeks, then 4 mg, with 6 mg allowed after at least 4 more weeks; maintenance is 4 or 6 mg. A news repost of the weight-management label gives the same steps. A phase 2 trial used the same 2, 4, 6 mg steps, each 4 weeks long. Higher-dose trials went to 9 mg in China and 10 or 16 mg in the US.

What doses of mazdutide were studied?

Phase 3 trials in China used 4 and 6 mg (GLORY-1, diabetes trials) and 9 mg (GLORY-2). Phase 2 trials tested 3 to 6 mg and 9 mg in China, and 3 to 6 mg, 10 mg and 16 mg in the US. All were once-weekly injections under the skin.

How much weight do people lose on mazdutide?

In GLORY-1, average weight change at 48 weeks was -11.00% on 4 mg and -14.01% on 6 mg, vs +0.30% on placebo. In GLORY-2, 9 mg gave 16.65% at 60 weeks vs 1.50%. In the US phase 2 trial, 16 mg gave 18.1% at just 32 weeks.

Mazdutide 9 mg: does a higher dose work better?

Yes, and it costs more side effects. GLORY-2 at 9 mg reached 16.65% weight loss at 60 weeks, but vomiting hit 53.1% and nausea 46.9%. In the US trial, 16 mg gave the most loss and also the most dropouts (20%).

How much bacteriostatic water do I add to a 10 mg mazdutide vial?

One guide uses 3 mL for about 3.33 mg/mL, where each U-100 unit is about 33 mcg. Another common choice is 2 mL for 5 mg/mL, where each unit is 50 mcg. The approved Chinese pen needs no mixing.

What if I miss a mazdutide dose?

As reported from the label: if 5 days or fewer have passed, inject as soon as you can and carry on. If more than 5 days have passed, skip it and take the next dose on schedule. Keep doses at least 5 days apart.
