Erections in men, subQ phase 1/2 trial
Who: Healthy men and men with erectile dysfunction who did not respond to Viagra.
Note: Doses above 1.0 mg gave a significant erectile response. Men with ED got 4 or 6 mg. Never taken to approval for men.
Also known as: Bremelanotide · Vyleesi · PT141

Who: Healthy men and men with erectile dysfunction who did not respond to Viagra.
Note: Doses above 1.0 mg gave a significant erectile response. Men with ED got 4 or 6 mg. Never taken to approval for men.
Who: Healthy men and men with erectile dysfunction.
Timing: First erections came at around 30 minutes.
Note: Only doses above 7 mg produced significant erections. Trial arms were 7.5, 15 and 20 mg. 7.5 mg nasal plus 25 mg sildenafil beat sildenafil alone in a 19-man crossover. Nasal per-spray amounts on the market vary by device.
Who: Premenopausal women with acquired, generalized hypoactive sexual desire disorder (HSDD). Not for men or postmenopausal women.
Timing: At least 45 minutes before sexual activity. Inject into the abdomen or thigh.
Note: Stop after 8 weeks if desire has not improved. In the phase 3 RECONNECT trials people used a median of 10 doses over 24 weeks, most 2 to 3 times a month and no more than once a week; the trial cap was 12 a month.
Who: Women using research vials. Every guide we checked repeats the Vyleesi label dose.
Timing: At least 45 minutes before sexual activity.
Who: Men. No dose is approved for men; guides extrapolate from the label and early trials.
Timing: 1 to 2 hours before sexual activity.
Note: One guide starts at 1 mg. Another notes a study where 1.25 mg still worked, as a lower option if side effects bite.
Who: First-time users checking how they handle nausea.
Note: One guide uses 1 mg as a test dose; another mentions 0.5 mg as a starting point for vial users.
Who: Self-reports on r/Peptides and in one site's dose logs, mostly men, often already on TRT or a PDE5 inhibitor, plus couples using it for planned nights.
Timing: Onset of a couple of hours at most, per one r/Peptides user.
Note: Logged doses cluster at 2 to 3 mg, with a range of 0.1 mg to over 5 mg. About 2 mg is called the usual dose; some drop to 1 mg because it still works. Start very low is the standard advice after bad first experiences.
Note: Unsourced, and it clashes with the label's 8-a-month limit. Label data shows dark skin patches in 38% after 8 daily doses in a row, versus 1% at label use.
Who: Men with erectile dysfunction who did not respond to sildenafil.
Timing: 45 minutes to 2 hours before sex.
Note: 33.5% responded vs 8.5% on placebo. The paper now carries a 2023 expression of concern, so weigh it lightly.

2 mL in a 10 mg vial is the most common mix, giving 5 mg per mL, so the 1.75 mg label dose is 0.35 mL. That is more than a 0.3 mL syringe holds, so use a 0.5 or 1 mL syringe. The Vyleesi pen is premixed and needs no water.
The FDA label dose is 1.75 mg injected under the skin of the belly or thigh, at least 45 minutes before sex. That is the Vyleesi autoinjector. Guides for research vials repeat the same 1.75 mg.
No dose is approved for men. Guides say start at 1 mg and go up to 2 mg if needed. Community dose logs cluster at 2 to 3 mg. Early trials found erections improved above 1 mg subQ, and gave men with ED 4 or 6 mg.
The label allows no more than 1 dose in 24 hours and does not recommend more than 8 doses a month. In the phase 3 trials most people used it 2 to 3 times a month.
2 mL is the common choice, giving 5 mg per mL, so 1.75 mg is 0.35 mL. Guides also use 1, 2.5 or 3 mL. Use the calculator for your exact vial and dose.
