Who: 1,070 patients with acute ischemic stroke in Asia, treated within 12 hours, plus aspirin.

Note: Trial dose: 30 mL a day, which is 6456 mg of peptide preparation at 215.2 mg/mL. The primary endpoint was neutral; a post hoc trend favored Cerebrolysin in severe strokes.

Stroke rehabilitation trial (CARS)

Amount
6,456 mg
How often
30 mL once daily
Route
IV, ready-to-use vial

Note: Trial dose: 30 mL a day, which is 6456 mg of peptide preparation at 215.2 mg/mL. The abstract does not state the route; the label sends volumes above 10 mL to infusion. Arm function was better at day 90; the authors call the trial exploratory.

Moderate to severe brain injury trials (CAPTAIN)

Amount
2,152 to 10,760 mg
How often
50 mL a day for 10 days, then two more 10 day cycles at 10 mL a day
Route
IV, ready-to-use vial

Who: 185 patients with moderate to severe TBI (Glasgow Coma Score 6 to 12), on top of usual care.

Note: Trial dose: 50 mL a day (10760 mg) for 10 days, then 10 mL a day (2152 mg) in two more 10 day cycles, at 215.2 mg/mL. The abstract does not give the gap between cycles or the route. Small-to-medium benefit at days 30 and 90, with safety similar to placebo.

Alzheimer's dose-finding trial

Amount
2,152 to 12,912 mg
How often
5 days a week for 4 weeks, then 2 days a week for 8 more weeks
Route
IV, ready-to-use vial
Cycle
12 weeks

Who: Patients with moderate to moderately severe Alzheimer's disease, in arms of 10, 30 or 60 mL.

Note: Trial arms: 10, 30 or 60 mL per dose, which is 2152, 6456 or 12912 mg of peptide preparation at 215.2 mg/mL. Each dose was diluted to a 100 mL infusion. All three doses improved global function at week 24.

Injected (ready-to-use vial)2

Nootropic IM course (community)

Amount
1,076 mg
How often
5 mL IM into the buttock, once daily
Route
IM, ready-to-use vial
Cycle
20 days
Rest
1 to 2 months; repeated 2 to 3 times a year

Who: Healthy adults using it as a nootropic; not studied in this group.

Note: Community dose: 5 mL a day, which is 1076 mg of peptide preparation at 215.2 mg/mL. 5 mL is the label's maximum for one IM injection. Some users go to 10 mL split between two sites. One MS forum member reports 5 mL IM about 5 times a week, in cycles with breaks.

Higher IM course (community)

Amount
2,152 mg
How often
10 mL IM once daily, split between two sites
Route
IM, ready-to-use vial

Who: Healthy adults using it as a nootropic.

Note: Community dose: 10 mL a day, which is 2152 mg of peptide preparation at 215.2 mg/mL. The label caps a single IM injection at 5 mL, which is why users split it. The source gives no course length for this dose.

Injected1

Research-vial subQ ramp (community, via guides)

Amount
20 to 32 mg
How often
Once daily, split morning and evening once a dose passes 1 mL
Route
SubQ, vial (powder)
Cycle
8 to 12 weeks, optionally up to 16

Who: Research powder sold by mg, not the pharmaceutical solution.

Week 328 mg140 units · 1.4 mL(2 draws)
Weeks 4 to 1232 mg160 units · 1.6 mL(2 draws)

Note: Amounts are mg of a 60 mg research powder mixed with 3 mL of water (20 mg/mL), not mL of the drug. One guide calls this ramp community-derived. These amounts are roughly 100 to 300 times below clinical courses and come from no trial. The pharmaceutical is not meant for insulin-syringe subQ dosing.

Mixing Cerebrolysin

Diluent: Bacteriostatic water (research powder only)

Water volumes sources use: 3 mL

Only the research powder is reconstituted. The pharmaceutical is ready to use; above 10 mL it is diluted in 0.9% saline, Ringer's or 5% glucose (to at least 100 mL total per the maker's card), all tested compatible over 24 hours. Never mix it with balanced amino acid solutions, lipid solutions or anything that shifts pH.

If you miss a dose

Do not double the next dose. Courses are planned as a set, so a missed day is a question for the treating clinician.

Cerebrolysin dosage questions

What is the Cerebrolysin dosage?

By the Austrian label: 10 to 30 mL a day for dementia (4 week courses, 5 days a week), 20 to 50 mL a day for ischemic stroke (10 to 21 days), 30 to 50 mL for hemorrhagic stroke (10 to 21 days) and 20 to 50 mL for brain injury (7 to 30 days).

What is the Cerebrolysin dosage in mg?

The drug is dosed in mL at 215.2 mg per mL, so 5 mL is 1,076 mg, 10 mL is 2,152 mg, 30 mL is 6,456 mg and 50 mL is 10,760 mg. Research powders sold by mg (20 to 32 mg a day in one guide) are a different product and far smaller.

Does Cerebrolysin need bacteriostatic water?

No. The pharmaceutical is a ready-to-use solution. Only research powders sold by mg are mixed, typically 60 mg with 3 mL of bacteriostatic water.
