Somatropin (HGH)

Also known as: Somatropin · HGH · Human growth hormone · rhGH · Recombinant human growth hormone · Growth hormone · HGH 191aa

Updated 35 sources

Recombinant human growth hormone: the full 191-amino-acid hormone, FDA approved since 1987 for children's growth disorders, adult GH deficiency and HIV wasting. Most people searching HGH want it for anti-aging, fat loss or bodybuilding, which is off-label, and US law makes distributing it for those uses a crime. Label doses are in mg; street vials are in IU, at 3 IU per mg.

What people use Somatropin (HGH) for

  • Approved: growth failure in children and adult growth hormone deficiency
  • Approved: HIV-associated wasting (Serostim)
  • Off-label: anti-aging and wellness at 1 to 2 IU a day
  • Off-label: fat loss and recomposition at 2 to 4 IU a day, often during a GLP-1 cut
  • Off-label: bodybuilding at 4 to 8 IU a day, usually with steroids
  • Off-label: joint, tendon and sleep recovery

How it is thought to work. Somatropin binds growth hormone receptors throughout the body. The liver answers with IGF-1, which drives most of the growth and protein-building effects, while GH itself breaks down fat and works against insulin, which is why blood sugar can rise. Injected GH replaces your own pulses: rising IGF-1 switches down the pituitary's own output.

  • Poor sleep or night waking with evening doses

Repeated reports; some users sleep fine on bedtime doses

    How people manage it

    • Many switch to a morning injection.
  • Fatigue, sleepiness, brain fog

Reported at higher doses (6 to 10 IU)

  • Rising fasting glucose and IGF-1 on bloodwork

Reported even at 1 to 1.5 IU a day

    How people manage it

    • Users test IGF-1 and fasting glucose before starting and a few weeks in, and hold the dose if they climb.

Who should avoid it

  • Active cancerCondition

Contraindicated on the label; monitor anyone with a past tumor for regrowth.

  • Acute critical illnessCondition

Two ICU trials in non-GH-deficient adults found higher death rates (42% vs 19%) on somatropin.

  • Severe diabetic eye diseaseCondition

Active proliferative or severe non-proliferative diabetic retinopathy is a contraindication.

  • Prader-Willi syndrome with severe obesity or breathing problemsCondition

Sudden deaths have been reported in these children.

  • Closed growth plates (for growth)Condition

Somatropin cannot add height once epiphyses have fused.

  • Insulin and other diabetes drugsSubstance

GH raises blood sugar, so doses of insulin or diabetes pills may need adjusting.

  • Oral estrogenSubstance

Women on oral estrogen may need larger somatropin doses.

  • GlucocorticoidsSubstance

Somatropin can unmask low cortisol; replacement doses may need to rise, and high steroid doses blunt growth in children.

Evidence and legal status

Evidence: strong human trials

Strong for approved uses: decades of randomized trials in GH-deficient children and adults. For off-label use, systematic reviews found healthy older adults lost about 2 kg of fat and gained about 2 kg of lean mass with more edema, joint pain and carpal tunnel, and athletes gained lean mass without more strength. Neither review supports anti-aging or performance use.

FDA and WADA

FDA approved as Genotropin +7

Approved by the FDA, but for a narrower use than people take it for here.

FDA approved (initial US approval 1987) as several brands, for children's growth disorders, adult GH deficiency and HIV wasting. Anti-aging, fat loss and bodybuilding are not approved uses, and 21 U.S.C. 333(e) makes distributing hGH for them a federal crime.

WADA bans growth hormone, its analogues and fragments at all times (S2.2.3). Long-acting relatives lonapegsomatropin (Skytrofa), somapacitan (Sogroya) and somatrogon (Ngenla) are separate molecules. Somatropin is a licensed biologic, not a 503A bulk compounding substance.

Somatropin (HGH) questions

What is HGH?

Human growth hormone. As a drug it is somatropin, the 191-amino-acid hormone made by recombinant DNA with the same sequence as the pituitary's own, sold as Genotropin, Norditropin, Omnitrope, Humatrope and other brands.

Is HGH a peptide or a steroid?

Neither in the usual sense. It is a protein hormone of 191 amino acids, far larger than research peptides, and it works through the GH receptor and IGF-1, not the androgen receptor.

What is the normal HGH dose for adults?

For adults with diagnosed GH deficiency, labels start at about 0.2 mg a day (0.15 to 0.3 mg) and adjust every 1 to 2 months by IGF-1. Off-label, community ranges are 1 to 2 IU a day for wellness, 2 to 4 IU for fat loss and 4 to 8 IU for bodybuilding. In mg those community ranges run from 0.33 to 2.67 mg a day (3 IU per mg), well above the label starting dose.

How many IU are in 1 mg of HGH?

3 IU. That is the labelling convention for somatropin, which is why a Humatrope 6 mg cartridge is also labelled 18 IU.

How many units is 2 IU of HGH?

With a 10 IU vial and 1 mL of bacteriostatic water, 2 IU is 20 units on a U-100 insulin syringe. With 2 mL of water it is 40 units. Use the calculator for your own vial.

Where do you inject HGH?

Under the skin of the thigh, buttocks or abdomen, per the Genotropin label, rotating sites every day to prevent fat loss under the skin. Humatrope also lists the back of the upper arm.

What is the half-life of HGH?

About 2.5 to 4 hours after a subcutaneous shot, depending on the brand, and about 0.4 hours given IV. The SC figure is longer because it absorbs slowly from the injection site.

Does HGH need to be refrigerated?

Yes. Brand labels say 2 to 8 C before and after mixing, protected from light, and never frozen. A few products allow limited room-temperature time, such as Norditropin FlexPro for 3 weeks in use at up to 25 C.

How long does mixed HGH last?

It depends on the product: 28 days for Genotropin and Humatrope cartridges, 21 days for an Omnitrope vial, 14 or 28 days for Zomacton vials depending on the diluent. One guide gives 14 to 21 days for generic vials in bacteriostatic water.

What are the side effects of HGH?

In adult trials the most common were swelling of hands and feet, joint pain, limb pain, tingling and stiffness, usually early and eased by a lower dose. Labels also warn about high blood sugar, carpal tunnel, raised skull pressure, thyroid and cortisol changes and pancreatitis.

Should I take HGH in the morning or at night?

No trial has compared them for body composition. Many Reddit users moved to morning shots because evening doses woke them at night; others dose at bedtime with no problem. Above 3 to 4 IU, people commonly split the dose.

Does HGH work for anti-aging?

Only modestly, at a cost. A review of trials in healthy older adults found about 2 kg less fat and 2 kg more lean mass with no weight change, plus more edema, joint pain, carpal tunnel and breast growth. The authors concluded GH cannot be recommended for anti-aging.

Does HGH build muscle or strength in athletes?

Trials in young, fit adults found more lean mass but no gain in strength or exercise capacity, and more swelling and fatigue.

How do you get HGH prescribed?

It is prescribed for diagnosed conditions on the label: GH deficiency in adults or children, children's growth disorders, and HIV wasting. US law bars distributing it for other uses, even with a doctor's order.

Is HGH banned in sport?

Yes. WADA prohibits growth hormone, its analogues and fragments at all times.

HGH vs peptides like CJC-1295 and ipamorelin: what is the difference?

Peptides make your pituitary release its own GH in pulses, capped by the body's feedback. Injected HGH supplies the hormone directly, gives a larger effect and suppresses your own production.
